INSTRUCTIONS

Thank you for your inquiry. To apply for trade pricing, please complete the 3 steps below. Incomplete applications cannot be processed. Applications are subject to approval by Cardstock Warehouse.

Wholesale Accounts and Sales Tax Exemption is available to:

1) businesses who purchase product for direct use in the production of products for resale.

2) Non-profit organizations purchasing product for use and not for resale.

3) Municipality or Government Organizations purchasing for use and not for resale.

4) Other approved use as determined by your state or commonwealth.

STEP ONE:

Create an account with us if you do not already have one. Be sure your account information is accurate and up-to-date at all times.

STEP TWO:

Complete the application form and click the SEND button at the bottom of the form.

STEP THREE:

The Commonwealth of PA requires completion of Form rev-1220. Please save this form on your computer, complete the form and upload it on this page.

Please note that this certificate must be submitted separately from the form on the right.

Acceptable attachment file types include: PDF, JPEG, PNG, TIFF

Once all of the above is completed and your paperwork received, your application will be processed as soon as possible, and your account will be updated for tax exempt status and the Wholesale Discount.
